Hours Our office hours are 8:00 - 5:00 Monday through Thursday, 8:00 - 3:00 on Friday and 8:00 - 12:00 one Saturday a month. Making an Appointment You can make an appointment by using the Contact Us button, or by calling the office at 815-962-0633. Appointments Office visits are by appointment only, although we try to leave plenty of open space in the schedule to allow for urgent visits that are needed the same day. If you are more than 20 minutes late for your appointment, we will try to work you into the schedule. Out of courtesy to our other patients, we may ask you to reschedule the appointment. After Hours and Emergencies The answering service is available with the doctor on-call by calling our office number, 815-962-0633. Only call the on-call doctor for urgent matters or problems. Routine questions should be saved for regular office hours, or by using the Contact Us button and sending an email message through the “NeedMyDoctor” link. Always call “911” for true emergencies. Prescription Refills Please request prescription refills by contacting your pharmacy. If you are out of refills, they will contact our office by fax and we will respond by the end of that business day, if not sooner. Please do not call after hours for routine prescription refills. Billing and Payment We expect payment of your copay at the time of service. There is a significant discount for self-pay patients and for HSA patients who pay at the time of service. For your convenience, we accept the following credit cards: VISA, MasterCard, Discover.
